Checking Swasthya Sathi Card Status by URN Number To verify the status of your Swasthya Sathi Card online using the URN number, follow these steps:

1. Visit the website.
2. Navigate to the MIS section on the left side of the website.
3. Click on the “Card Verification” option to check the Swasthya Sathi Card status using the URN number.
4. Select “West Bengal” as the state.
5. Choose your district.
6. Enter your Swasthya Sathi Card URN number.
7. Submit the form, and your Swasthya Sathi Card status will be displayed.

Checking Swasthya Sathi Card Status by Application Number or URN Number

If you wish to check the Swasthya Sathi Card status using the application number, you can visit the “Card Verification” page since the URN number is the same as the application number.

Checking Swasthya Sathi Card Status by Aadhar Number

If you cannot recall your URN number, you can still check the Swasthya Sathi Card status using your Aadhar number and registered mobile number. Follow these steps:

2. Click on the “Find Your Name” link from the menu.
3. On the next page, enter your registered mobile number.
4. Select the type of Swasthya Sathi Smart Card.

8. Enter your Aadhar Number to search for your Swasthya Sathi Card.
9. Submit the form, and your smart card status will be displayed.

4. Click on the “Search” button to check the hospital empanelment status.
